The immediate postoperative period following neurosurgery is one of the most critical phases of recovery. During this time, perianaesthesia nurses play an essential role in recognising subtle changes that may indicate neurological deterioration while supporting a safe transition from anaesthesia to recovery.
Neurological assessment begins the moment the patient arrives in the post anaesthesia care unit. Airway, breathing and circulation remain the priorities, but they are closely linked with protecting cerebral perfusion and preventing secondary brain injury. Even small changes in consciousness, pupil responses, respiratory pattern or blood pressure can signal significant neurological complications. Frequent observations and comparison with the patient's baseline are essential.
💙 Maintain a patent airway while minimising coughing, straining and hypoxia, as these can increase intracranial pressure.
💙 Perform regular neurological assessments, including Glasgow Coma Scale, pupil size and reactivity, limb strength and changes in consciousness.
💙 Closely monitor blood pressure, oxygenation and ventilation to support adequate cerebral perfusion and reduce the risk of secondary injury.
💙 Monitor fluid balance carefully, recognising that electrolyte disturbances and diabetes insipidus may occur following neurosurgery.
💙 Position the patient with the head elevated approximately 30 degrees and maintain neutral head alignment to promote venous drainage and reduce intracranial pressure.
Effective postoperative neurosurgical care relies on vigilant assessment, timely recognition of deterioration and clear communication with the multidisciplinary team. Small changes can have significant consequences, making ongoing observation one of the most valuable nursing interventions.
Perianaesthesia nurses are uniquely positioned to identify early neurological changes and intervene promptly. Careful assessment, thoughtful clinical judgement and consistent monitoring contribute to safer recovery and improved outcomes for patients following neurosurgery.
