Just culture and building a safety culture

Every perianaesthesia nurse has had that moment of hesitation, wondering whether speaking up about a near miss is worth the risk. Just culture gives us the answer. It tells us that reporting errors and system weaknesses is not a weakness in ourselves, it is a strength in our care.

Just culture sits between two extremes. One extreme punishes every mistake, which drives people to hide errors. The other ignores accountability altogether, which erodes standards. A true just culture finds the balance, supporting individuals while still expecting honesty and responsibility from everyone involved. It recognises that most errors happen because systems allow them to happen, not because someone was careless.

This matters deeply in perianaesthesia care, where handovers, medications, and rapid decisions happen constantly. A culture that invites honest reporting helps teams catch patterns before they become patient harm.

Key elements of a just and safe culture include:

🔹 Consistent concern about what could go wrong
🔹 Respect for expertise, regardless of job title
🔹 Willingness to adapt when situations change unexpectedly
🔹 Flattened hierarchy when patient safety needs it
🔹 Honest reporting without fear of blame

Practically, this means perianaesthesia nurses are encouraged to report near misses, question unclear orders, and flag hazards early, knowing the goal is learning rather than punishment. It also means supporting colleagues after an error, rather than assuming fault, while still expecting honesty and accountability from the whole team.

Building a safety culture is not a one-off project. It is a daily commitment to noticing, reporting, and improving together, so that patients remain safer with every shift.
